If during the first stage, that is to say, during the fever, the patient should have an intolerable throat ache, the pulse beats forcefully and full, [and] breathing is labored, give to drink the juice of sour pomegranate, or of *alibukharu* [prunes], or of lemon, or of barberry, mixed with water, with sugar or honey; [and] stick sour dough kneaded with mustard to the soles of the feet. If there should be constipation in the bowels, give a purgative medicine under Number 4, or 5, or 6, or 7, or 8; or have [the patient] take through the rectum a [piece of] soap carved to the thickness of a finger and rubbed with oil, or administer a mild clyster with the medicine written under Number 20. 82 [83]. If the patient should happen to suffer faintness of heart, numbness should come over him, and his whole body should tremble, use the remedies written in the article above. If during this time the patient does not allow medicine near [him], give a clyster, or have [him] take soap through the rectum, and stick sour dough with mustard to the soles of the feet and leave [it] on for a long time, and frequently have [him] smell vinegar. 84. Before the smallpox erupts, if the skin of the patient's body is dry and parched and hinders the smallpox from erupting easily, moisten a piece of linen or sponge (*siwnkēr*) in warm water, rub [it] over the whole body, and make [him] drink the medicine under Number 10. 85. At the time when the smallpox is erupting, if the patient's fever cools down entirely, but he himself is very weak, at that time they must give the strengthening medicine under Number 13 or 14; or make [him] drink good wine mixed with water, one spoonful at a time.
